1. Identification of Suicidal Ideation
During the review of the initial questionnaire, it is essential to pay special attention to the client's responses regarding suicidal ideation.
If a client responds "YES" to any questions indicating suicidal thoughts, these responses are immediately flagged for further evaluation.
2. Decision-Making Process
Upon identifying suicidal ideation in a client's questionnaire, you, as the therapist, have the option to make an informed decision:
If the response indicates a moderate or high risk, you may choose to involve our medical supervisor onboard for additional assessment.
Depending on the severity of the client's response, you may decide whether to proceed with the therapy session or prioritize the client's safety by scheduling them for a comprehensive evaluation first.
3. Scheduling and Further Assessment
When clients require additional assessment, our medical supervisor will be actively involved in the process:
The client will be scheduled for an appointment with the medical supervisor to conduct a thorough evaluation of their current situation.
As part of this assessment, the client will be asked to fill out the 'Member Medical Data' form to provide relevant medical information for a comprehensive evaluation.
Accessing the Medical Questionnaire
To review the client's medical-related information, follow these steps:
Click on the 'Members' tab in your Wellnite Help Desk account.
Locate the client whose Medical Questionnaire you wish to access and click on 'View' to open their profile.
Within their profile, navigate to 'Clinical Information.'
Under 'Clinical Information,' select 'Medical - Member Medical Data' to access the client's medical-related information.
