Step 1: Provide Your New License Details

Kindly send us the details of your newly upgraded license. We require the following information:

    • Please provide the title or type of your new license (e.g., Licensed Psychologist, Licensed Clinical Social Worker, etc.).

  2. State of Licensure

    • Let us know the state or jurisdiction where your new license has been issued.

  3. Issue Date

    • Include the date your new license was issued.

  4. Expiration Date

    • Provide the expiration date of your new license.

Step 2: Verification Process

We take the verification process seriously to ensure the accuracy and validity of all licenses displayed on our platform. Our team will review the information you provide and perform the necessary checks. Please allow a few days for the verification process to be completed. Rest assured, we work diligently to process updates promptly.

Step 3: License Update and New Contract (If Applicable)

Once your new license has been verified, we will update it in our secure database. Your updated license will now be visible on your Wellnite provider profile, showcasing your latest qualifications to potential clients.

If applicable to your specific provider agreement, we will also send you a new contract reflecting the changes in your credentials. This updated contract will ensure that you are appropriately represented as a licensed professional on our platform.
